cant get it right!

Hi,

My name is Zoe, im 24 and was diagnosed with type 1 diabetes in october 2006. At first i found the condition easy to control (once i was on track), i took 16 levemir at night, no insulin with breakfast, 5 novorapid at dinner and 12 at tea (give or take). I worked with horses at the time and so was very active and hardworking.

In the following june i started to work as a veterinary nurse at a busy animal hospital. this involved shift work, a 45min journey to and from, long hours and lots of stress. As you can imagine I struggled, particularly when doing the night shifts. my blood glucose was either much too high or much too low, no pattern to it despite my efforts to find one.

so, in janurary i changed jobs to work at a much smaller local practice. this was better at first, and although far from perfect, my blood sugar was better and less erratic. after the honemoon period, the vet and manager started to put alot of pressure on me, i couldnt work hard enough and was always in the badbooks for something. my blood sugar then went erratic again, this time giving very high readings regularly. at work they were very unsypmathetic and not understanding at all. I also had more stress as my partner was suspended from work for three months. I ended up taking time off sick, as the doctor said i needed to get rid of some stress before i tried to control my diabetes. i finished working here in may.

I started to work for myself then, i work with horses again, and also do pet care services, dog walking etc. im so busy but I love it and best of all, no stress!

Over the months i slowely adjusted my insulin. i seemed to get it right, my blood glucose would be fine for a week or so, and then the readings would go higher again. this repeated itself many times.

it finally settled around october. i had a whole two months with nearly perfect blood glucose levels, the odd blip but nothing major. my insulin was now 36 levemir at night (up from 16!), if working no insulin with breakfast or dinner, if off work 10 with breakfast and 14 with dinner. 16-20 units with tea.

all was well untill christmas when i had a well earned rest and then my bg increased (regular readings of 15-18 for 2 weeks, even waking up at these levels). this was very disheartening as you can imagine, i thought i'd cracked it. now im working again its fine but i cant help feeling i havent got it right yet. its ok if im working really physically hard, but i cant seem to have a rest without it rising again. and i feel like im taking huge amounts of insulin is this normal? also, despite all the mucking out and walking i do i cant lose weight. i only have toast for breakfast, and a banana and cereal bar for dinner, and then a healthy tea and a bedtime snack. im 5ft3 and weigh 12st5!!!

Hi Zoe and welcome.

It occurs to me that your breakfast and lunch are both carbohydrate oriented with little or no protein. You don't say what sort of thing you have for your "healthy tea" or bedtime snack, but are they also high in carbs?

As I'm sure you already know our sugar levels are influenced almost totally by what we put in our mouths. Have you tried substituting the breakfast toast for a couple of eggs (without bread). Or try having a meat or cheese or tuna salad for lunch - cereal bars are usually not only high in carbohydrate but also very high in sugar.

Of course you would probably need to adjust the insulin doses to compensate for the reduction in carbs (I'm a type 2 so not really qualified to help you there).
