I am looking for a set of chronological tasks to use at my GP and get it on to their web site. I am looking for a simple guide to the visits at each stage of the visits for example Annual check up and 6 monthly check up
1. Visit GP for XYZ Drugs review or XYZ checks
2. Visit visit for Blood tests ( test x and y and Z) and Urine tests for XY Z -
3. Return to doctors for Blood test Results x days after blood tests.
4. Visit for feet checks, blood pressure
5. Eye checks and Photograph of the back of the eye.4
Other checks that may be required that are outside the regular routine such as Insulin review with a hospital specialist.
I am looking for the right chronological order and the correct tests and checks as I can find a simple list of these with a patient friendly explanation as an addendum.
