This is an Australian Initiative for people in Victoria but as most of the posters here are from the U.K. we would be unable to access it but thanks for sharing. We do have similar risk assessments with DUK.

if once you are in relation with cadiovascular disease,then you won't be able to get back from there...if not 100% otherwise!!!
