I know this was asked 100 times already, but I never got it clear...

If I'm getting a day off work and I basically spend most of the day in the hospital (so I can't go back to the office for half a day or something), do I have to take this as a day off (ie from my holiday allowance) or is there a law saying that it's like an "extra" day off? Or is it at employer's "discretion"?

As far as I'm aware there's no obligation for your employer to give you the time off paid, so it is at their discretion if they pay you or not. I would say most employers will let you take it as a lieu day or might even might pay you provided you make the time up.

Some not all employers will allow employees to take time off for tests if it is work related, ie; eye test for a fork life licence, or glasses for computer work,
it depends on the employer I would ask your employer if he/she runs such a scheme.
Some employers will cover any test for anything.
If in doubt take a days holiday or you could fall foul of The Bradford factor. google it

Your employer must give you reasonable time off to attend appointments for your diabetes, but is not obliged to pay you for the time he allows you off.

Regarding the rest of the day query, that's a very grey area. Is there any reason you couldn't go back to work? Or is it just that going into work mid-afternoon just doesn't feel worthwhile?

If you do flexi time, you may be able to do a deal with your line manager on how to handle it, otherwise, it could be worthwhile explaining the situation to your manager and see what he/she suggests. You may be able to make the time up over a couple of weeks, say, rather than have to take a half day holiday. I dare say it could depend on ow often this is likely to occur and if anyone else might be impacted by either your absence or any actions your manager might take.
