Type of diabetes
Type 2
Hi.... I've posted before but I was treated for T2 back at the start, I've since had very high hba1c at 86, 94 and 98. Had another blood test done to look at "what type of diabetes I have" looking for markers in antibodies????? Anyways, long story short, I WS started on insulin regime, (after years of a cocktail of oral tablets were unsuccessful- I'm currently on 18units x3daily of novarapid and 26 units of lantus) .... So.... Conversation with nurse specialist a couple of days back, and she suggested the following "I see that you have an appointment with the consultant soon, I'd like you to ask him to explain the TYPE OF DIABETES you have, as your markers were just on the border, but your insulin in reserve was,..... Well, it was...... Anyway, yiu need to speak to the consultant, it's not going to change anything, you're still going to need insulin ". Now is it just me, or did that message appear to be telling me something that I have clearly missed?? I was told I WS T2 for years, told to take these array of pills, vomited and felt ill for months, but" percevered ". I then got told I was late onset T1, started insulin, stopped all oral tablets, now I'm hearing that the type I was told may not be right?.... My BG is not responding as well as I would expect from insulin regime. For example my BG yesterday at 4pm before food was 23. 7. I average between 19 and 26 and have done for 2years. I keep away from the bad stuff (carbs) and I keep a strict food log. My life evolves around this bloody BG situation. I'm still very very tired, blurry vision, and bruised tummy.... Basically I'm having a blooming yukky time. (sorry for the rant). Why would I have to speak to my Dr about Antibodies?? What did she mean about markers? And why can't I get my BG below 15 let alone 10?

Either you are antibody positive, or you aren't. It's a yes or no question. So if your results are showing you as "borderline" for the antibody tests then that would suggest you are antibody positive and type 1. You could ask for an explaination for what antibodies have been screened for and what the results were.

I'm not sure what the nurse was talking about with "insulin in reserve", possibly a cpeptide test? It's not unusual for type 1s to have residual insulin production, this is what the honeymoon period is.

If you can't get you blood sugars down then it sounds like your insulin regime needs adjusting. You lantus should be keeping your blood sugar stable without food or bolus doses. You can do some basal testing to see if it is or whether an adjustment to the 26 unit dose is required - https://mysugr.com/basal-rate-testing/

Your novorapid should be covering your food and correcting highs. Most type 1s are carb counting, you seem to be on fixed doses. You should ask for a place on a DAFNE course or do the free NHS online course to learn about carb counting - https://www.bertieonline.org.uk . Your fixed doses of novorapid will only be able to deal with a certain quantity of carbs/protein with meals, if you exceed what they can cope with, you'll go high.

If you're having results in the 20s you should be testing for ketones and correcting. You should seek guidence on your correction dose.
