Your blood sugars are certainly still too high, so that points to one of two things. Either your insulin doses are too low, or your intake of carbohydrates is too high. Maybe a combination of those things.
4 units of Novorapid per meal is quite a low dose, although obviously dependent on what you eat. Can you tell us what your typical meals consist of? 12 units of Lantus perhaps isn't too far off a reasonable dose, but if your intake of carbohydrates is too high it probably won't be enough to keep your blood sugars down.
As a final point, a non-diabetic's blood sugars will hover around 5 mmol/l for most of the time. Long term, those are the sorts of numbers to aim for.
