Hi all,

I've been struggling with getting my morning dose right. I take 6 units of Novorapid for 45g carb (toast and oats).
After breakfast my sugar can rise by around 6 mmol (from 6.5 to 13) and will go back down to about 8 around 3 hours after. But if I take 1 unit more novorapid, my sugar level will drop quickly and I'll have to eat 10g more to make sure I don't go low. This is confusing as 6 units seems to insufficient, but 7 units is too much.
It seems that the peak can be eliminated by one extra unit, but then blood sugar will drop afterwards, so it's like a rollercoaster, which is not a good system.
Any one have any idea what I could be doing wrong? Could the breakfast be eaten more slowly to reduce that peak. Or could porridge be bumping it up and dropping.

Thanks for your help!
Thomas

It may be worthwhile asking for a half unit insulin pen to enable you to inject 6.5 units of insulin.
These are pretty common now.

Hi @thomc_82, In addition to @helensaramay 's excellent suggestion , can I ask what basal insulin you are taking?
I am just wondering if its action around breakfast time is having an influence on your BSLs in addition to your Novorapid?

Hi, thank you for your replies. I'll ask my doctor about those half-unit pens, sounds like that would be helpful.
My basal is 9 units of Levemir taken once a day at 20:00. My lunch time ratio is 40g carb to 4 units novorapid. My dinner ratio is 35g carb to 4 units novorapid. These ratios take into consideration general exercise etc. I don't know what my ratio would be for sedentary days; possibly +1 unit at all meals.

Thank you @thomc_82, So one possibility could be that the evening levemir is affecting your morning BSL in conjunction with the Novorapid.
A number of people on Levemir take it as a twice daily dose. That way there is more flexibility and in some persons the Levemir does not last 24 hours.
Perhaps speak with your DN or doctor to see whether a split dose of Levemir, am and pm, plus the 1/2 unit pens for the Novorapid doses can both help to solve the after- breakfast trouble.
