Just a though, not knowing the culture and atmosphere of your workplace - but a letter to said manager pointing out the the risk assessment is outstanding, and does he/she have any news as to when it might take place?
In which case, write and ask HR. Should you use email, my suggestion would be add a Read Receipt request and add yourself as a BCC to give you confidence that firstly it goes properly, then know it has been read (if they send a receipt).
Personally, I would also copy my Manager in too, then you are playing squeaky clean and can't be accused of going behind his/her back.
