Hello everyone!
I'm new to the forum and, therefore, should first introduce myself.
I'm Vithor, 20 y.o. and I'm from Brazil (so, excuse any English errors I may commit!). Well, I'm almost certain I have 1.5 Diabetes because my insulin production has been tested and diagnosed as non-existing AND that diagnose happened when I was just turning 15 years old. Also, my mom has multiple sclerosis (another autoimmune disease), so I have good reasons to say that. And I think my doctor has stated it as well.
Well, anyway, why am I writing this post? Basically, because my diabetes (type 1.5/type 1-ish) is spinning out of control. I used to be a fat kid when I was younger. Then, when I developed the diabetes, I lost a lot of weight and so on... So, I've been a diabetic for 5 years now and my weight tends to be around 90~95kg (198~210lbs) and I'm 1.85m high (about 6.06"). Don't mind the conversions, Brazil uses metric, so I thought it'd be nice to convert it before posting.
Anyway, let's cut to the chase: when I first started using insulin and also the diet, everything worked fine. In my first exame after three months of control (back when I was 15), my HBA1C was about 6%. And for about 3 or 4 years it was around that value: 5.5% ~ 6.5% maximum. I used to do a LOT of exercise (heavy weight gym exercises, with machines and free weight, around 5 to 6 times a week). Of course, in those 3 or 4 years, it was not ever that I was able to exercise. There were some months that I had to stop due to school obligations and so on and so forth (what I mean is: I wasn't an exercise freak, some months I went by completely using just insulin). Now, however, I'm having a lot of trouble to control it. I stopped the exercise, yes, but even when I tried to go back to the gym (and trust me, I've tried more than twice to go back, then I stayed a couple of months and gave up because of the lack of results), the insulin has had no improvement whatsoever.
I take 22 units of Humalog mix 25 (it's a mix, as the name says!, of 25% Lispro - fast - and 75% de Lispro Protamine - slow). I have used that insulin since the beginning of the treatment and hadn't had any problems with it up until now. My strongest belief has to do with the region of the application (the body part, I mean, and I hope I'm making myself understandable here!). Some days (and I eat the same food everyday on breakfast), I have hypoglicemia around 10a.m. (I eat 6a.m.) - although those days are really rare. And most of the days I don't have hypoglicemia (which either means I have hiperglicemia or it's stabilized). However, as my more recent exams show, my glicemia is rising: from the usual 5.5%~6.5% average, I now have 7%~8.5%. The thing is, how am I supposed to know where to apply the insulin if, for instance, one whole week I apply it on my thighs (both left and right) and some days the BS stays controlled and other days it gets completely out of control (200~350)? And, usually, my diet has been the same ever since the beginning of the treatment: I eat less carbs than the average person and more protein. I'm getting to the point of eating no carbs at all! I mean, 30g of carbs daily is TOO low in my opinion (I'm eating around 90g a day). Any advices? What should I do? I suggested my doctor to switch insulins. Now I'm taking Met 1000mg combined with 50mg Vildagliptin (it's called "Galvus Met 1000 + 50" here, I'm not sure if it's the same name in other countries).
