Diabetic Neuropathy - feet

Hi Everyone
I have been a diabetic T2 for over 20 years on Insulin. I'm 55 now and my goal to control my glucose levels has been somewhat of a bumpy ride, despite always having excellent NHS support.
Among other complications, I am suffering BADLY from Diabetic Neuropathy in my both my feet. I am taking Duloxotine 60mgs daily along with both Amytriptalyine 20mgs BD and another strong pain-killer Tramadol 50mgs PRN
MY QUESTION IS; CAN ANYONE EXPLAIN TO ME WHY MY FOOT PAIN IS SOOO PAINFUL IN THE EVENINGS AND THRU' THE NIGHT ?
The daytime pain, even having taken the medication, remains painful but bearable. But in the evenings . . . . It's off the chart !! Anyone else out there who has the same ? Would like to hear from you . . .
 
could it be that maybe the legs are slightly more swollen at night, like when people are told to buy shoes late in the day because normal feet are a little bigger at the evening than in the morning...?
but don´t know ...
or maybe also the exchausting of the nerves in the legs are greater in the evening than at mornings...?
 
Have you had your magnesium levels checked?

I know pain management teams for daytime try to make patients tjink of distraction techniques and mindfulness.. At night, especially when asleep how the heck can we try to use those techniques?

I found having alpha lipoic and magnesium supplements helped.

Despite pain I also try to ensure I do cycles whilst sat down on settee. Just 600 a day but it does help at night.

I also suffer from neuropathy and have had many sleepless nights because of it. My Consultant at hospital put me on Pregabalin and Tradorec XL (which is slow release Tramadol) and this helped a bit, but some nights were still hell. He then prescribed Lidocaine Plasters (Versatis 5% medicated plasters) which are for treating shingles. I now put one on the top of each foot at bed time and this gives me a full nights sleep, with some mild discomfort but no more extreme pain.
They work for me, so maybe worth a try?
