Passport? I think the receptionists are making up their own rules because they can't be bothered to give you the results. Do you also have to show your passport when you want to see the doctor? Ridiculous!
If you don't have a driving license take some correspondence with your address on it : a bill of some sort. I would be threatening a letter to my MP if it were me. I would also complain in writing. Perhaps your surgery might like to start taking photos of patients when they register so they can see instantly they have the right person. Wouldn't help with telephone but if they call you then they should be able to provide you with your results.

Life is fun when you have no photo id. My passport expired in 1992 so the photo would date to 1982 and is in my maiden name. I don't go abroad so I haven't renewed it. As I am partially sighted I can't drive, its always fun to see people faces when you ask them to ask for an id that doesn't discriminate against anyone with a disability.
 
Passport, just not true , you are entitled to a print out copy of your blood results ...
Ring the practice manager to discuss concerns over the way you are bring treated...if you feel,you are able to do that ...
Ask for an emergency appointment to see a GP ....you don't have to give a full reason to reception , just say urgent diabetic problems or again ask the practice manager ....

Most surgeries allow you to ring for results as well ....
