Hey all,

Can you have this as a type 2, for me I loose weight when my sugars are high and put weight on by looking at food when am on insulin. I’m having Bariactric surgery at the end of the year and am not allowed to put weight on but with insulin I put a stone on despite sticking to a strict low carb diet. But if I leave the insulin out I loose weight.

I wonder if @ickihun can help you, I know she’s recently had bariatric surgery and takes insulin.

Oh dear Absxx - My feeling is if you are manipulating your insulin doses to manage your weight, and you feel any frm of compelling reason to do it, then it describes diabulimia. That you don't fit the usual definition of it would be unusual, but obviously that takes absolutely nothing away from the dangerous nature of what you are doing.

As you likely know, the reason you are losing weight relates to yout high blood sugars. That in itself is dangerous, and should you persist with missing your insulin - aside from the danger to your health and well-being, you are at risk of tripping up with your sugery. Surgery of a non-emergency nature is not allwed to happen, with a high HbA1c. It becomes too risky.

I appreciate that you must feel in a hard place with all of this, but, you need to be safe, and well. Please do stop this activity and take your insulin. What insulin regime are you on? Do you take fixed doses of insulin, or do you vary, depending on what you eat and drink?

Finally, do you have test strips or uring strips to test for ketones? Testing for ketones when blood sugars are high is very important. The consistentlt high numbers, if I recall correctly is consistently over something like 13-14, although I'll tag my T1 colleagues @urbanracer and @Diakat , if they're around, as they'll surely know that number. This page talks about it a bit:

https://www.diabetes.co.uk/diabetes_care/testing-for-ketones.html

If you can test for ketones, the important numbers are:

I really would urge you to take your insulin as you should and discuss your findings about the weight gain with your diabetes, and bariatric team. You may need a tweak to your doses, or the type of insulin you are using.

Really, really good luck with it all. You must feel like you're backed into a corner.

Thanks for the tag @DCUKMod

@Absxx - hi there, really sorry to hear that you are facing this struggle. As DCUK mod says you need to test for ketones if over 14 for two consecutive tests. But ultimately you need to talk to your team about this - biatric surgery is no magic bullet or walk in the park, you will need to look at your relationship with your body before deciding this is right for you.
You may drop weight by missing the insulin but you are also harming yourself.
Huge hugs because this is clearly not an easy time for you.

Thanks everyone I’m taking my insulin have recently been put on Toujeo and Apidra. My bloods have been in the high teens for about 4 months. Until I saw the diabetes specialist nurse who only comes to my gp’s every 3 months. I was on humulin then the dark blue pen. The nurse knows my sugars have been high for that time but didn’t mention ketones. I am also on 1000mg bd of metformin too

Do you carb count and adjust doses or are you on fixed doses?

I’m on fixed doses so far
Well that could be part of the issue. I tho k it is worth you asking for a hospital consultation in light of high numbers and upcoming surgery. Adjusting insulin allows for better control and more flexibility in what you eat. You need more help than you have been getting.

Toujeo300 should hv been given with the view to reducing your basal insulin. How close are you to having any of the various bariatric surgeries @Absxx?
Are you under pressure to lose to be accepted by the surgical team?

What is your typical eating routine? That and insulin and exercise is what causes weight loss and weight gain in a type2.

I can lose on insulin (or gain). I was put on Toujeo300 for flexibility. It is a very helpful insulin when losing weight and reduction is needed to make room for more loss and reduction.
I was on 96units of Toujeo300. I've lost over 2st and now use 34units. I'm waking on 6s. If very little carb and not too much protein in my small tummy after Roux-en-y I stay in 5 and 6s.
I only lose weight on insulin if I'm in 5s and occasional 6s.
