Faulty infusion set or faulty insulin paradigm 754

I have been a type 1 diabetic for 18 years. I was switched over to a medtronic paradigm 754 pump just under two years ago due to my 'failure to get control of my diabetes'. I use the silhouette infusion sets but I have tried both the quickset and the easyset. I use humalog insulin from 10ml vials and change my site every two to three days with the reservoir and the infusion set.

Over the past two years I have had 5 unexplained occurrences of hypers directly after changing the infusion set. Generally I would notice the bg go up after feeling ill and change reservoir, insulin, infusion set and site and then it would be OK.

In an attempt to get my Hba1c under control I have taken to testing my bloods nearly every hour day and night since 3rd April which has been a real eye-opener and not recomended unless you are very interested and get the go ahead from your DSN. I am using a two carb meal one protein meal routine to get my basal right under the guidance of my DSN. Anyway today I had a carb breakfast about 8am and I changed my infusion set at 10.16am. I noticed that my bg was starting to go up. So ok, it could have been an incorrect basal or maybe my bolus ratio was wrong. Because I was doing hourly I could see the bg go up until I felt really ***** when it hit 11. So I changed everything and by the next hourly test my bg was back to 9 and in a further 2 hours was in zone at 6.9. Phew. But what was the cause? I emailed with my concern about the insulin. Every time it has happened before the vial I used was one that had previously been used. My DSN said it was more likely to be the site or the set than the insulin. I have since proved that it was not the site and could only have been caused by faulty insulin or a faulty infusion set.

Has anyone had anything similar happen to them or is it me who is the only unlucky one?

Hi mate, I've just concluded that I myself was using a bad insulin vial so I think it is possible. I was using novorapid and had unexplained high blood sugar levels for a few days. Firstly I put it down to stress, then I changed my cannula, then the reservoir itself. I then decided to increase my overall basal rates by 25% and I was still getting high blood sugar. I then started using a new insulin vial and everything was back to normal. It makes me wonder how often this happens.
