90mg Gliclazide a day is not a particularly high dose, and if its not doing the trick you should see your doctor to discuss increasing it. (Until recently I was on 160mg Glic twice a day). It works by forcing the pancreas to produce more insulin; unfortunately this solution doesn't last for ever as after a while the poor old pancreas begins to "wear out", and you either have to increase the Glic, or try another tack. (I'm now on Byetta, and the Glic has been reduced to 80mg twice a day.)
Everyone is different, so you need to dicuss with your doctor a strategy for dealing with your particular set of circumstances; the solution you come up with may be quite different from mine!
