HBA1C V Spot Spot Levels

Hello All,

Just after some views on blood glucose levels, I am a t2 diabetic diagnosed about 15yrs ago running on Lantus (since 2014) & Metformin, i think I am fairly clued up on the condition however i have had a real mixed response from my GP on hba1c and spot glucose levels and wonder if readers have any view.

My Hba1C ranges from between 6.2-7% in old money, I find I get some horrible spikes mainly after lunch, whereby levels are in the 10-12 range after 2hrs, fasting levels range from 5-7 depending in the night before.

My GPs of the view that i should ignore post and pre prandial results and focus on the hba1C result instead.

Personally, I don't take meds, but in your shoes, I would much prefer to flatten some of those peaks and troughs a bit. In situations as you had with your Doc, I find it useful to ask if that's what they would do, if they were me. It can be a telling moment.

I'm not suggesting for an instant your Doc is uncaring or knowledgeable. How could I know that? But, I do think sometimes we can all get carried away with looking at one measure, and for T2s in particular that seems to be the HbA1c, rather than looking wider at the whole picture.

I couldn't and wouldn't begin to suggest how you move forward from here, but I'm sure there will be plenty of folks with views and experience who may be able to help.
 
Yes its an interesting one, i have been asked to get referred to the clinic to discuss these spikes and look at perhaps a pre mix or fast acting insulin, however the clinic have rejected my referral on the basis that my Hba1c is within target range, so where to go from here ??
 
Initially, I would say that your GP is your gateway to a secondary care clinic, so I would go back and rediscuss it with him. If that didn't work, I would consider going up the chain.

I can never remember how those things should be escalated, but I'm guessing other will know.
