Help with Levemir

I am having a break from the pump as I need some treatment on my hand and wont be able to use it for a week or so. I have returned to MDIs using Levemir morning and night and Humalog as bolus. On the pump, my total basal rate was around 18 units.

I started with 10 units before bed and 8 units in the morning - I have now increased it to 14 units pm and 11 units am - but my readings are still too high especially late morning and before bed. I have upped my carb to insulin ratio a bit too. I am injecting in my thighs for levemir and stomach for humalog. For example, I tested at 9am this morning 9.7, took 11 units Levemir had no breakfast and at 11am - 14.5?? The day before, tested at 8.30am 11.8 took my Levemir, went for a 45 minute walk, no breakfast tested at 11am - 17.5..............

Before going on the pump I took Lantus (17-18 units) once a day before bed but am advised that Levemir twice a day can help with control.

Any ideas??

I also switched from Lantus once a day to Levemir twice a day before I got a pump. From what you have said in your msg, as your bg levels are a bit up in the morning, you probably need to increase the bedtime Levemir first of all so that you are a bit lower when you get up. Before you do this, are you able to go to bed with the same bg level? If not, then you will need to alter the carb ratio for your evening meal so that your bg levels are more or less the same each night when going to bed. That way, it's easier to find the correct bedtime basal dose. Once that is done you can then increase the morning basal up a tiny bit so that you are not having to use really large bolus doses. It is all trial and error really and test, test, test until you get your bg levels as you want them.

It might be because you had no breakfast. If my son eats nothing in the morning his blood sugar shoots right up. As long as he has some apple juice when he first gets up, followed by cereal which could be a good half hour later we keep his levels on track.

I THINK......If you have no breakfast your liver releases some glycogen for energy, which would increase your BG levels.
 
It's weird that I could be on the pump and miss breakfast with no BS rise but with MDI's it's completely different :?

I am now on a combined Levemir dose of 30 units (16 units pm - 14 units am) and on Lantus, pre pump, I was on 18u before bed and my basal rate on the pump was 17-18u..............bizarre!!??

My hand will be back in action at the weekend so I will go back to my Animas and then beg to be put on the Omnipod when it arrives in France.

I will need another break from the pump when the rest of my hand is treated in a couple of months but am thinking of asking for Lantus rather than Levemir and just keep the Levemir for emergencies such as pump breakdown................ Trying to be a good diabetic isn't easy :?
