I have been diabetic for 35 years ! Always had low sugars but have had reasonable control . Now suddenly I am having problems have been on my insulin since 1981 but seem to suddenly be having problems with major hypos I am on the novo pen at the moment but have been looking at at the accu-check combo system pump it's a big change but I am disabled and often have trouble doing injections and blood tests due to bad carpel tunnel that dr say I cannot have surgery on can any one advise me as to how I explain to my dr about needing an insulin change and how this would help me ?!!!

I got a pump after I started having severe hypos late mornings usually, after my 2nd child was born. I needed an ambulance calling twice, when I was out with 1 or both of my children. I'm sure the NICE guidelines say pumps are approved if a patient is having frequent severe hypos on multiple daily injections and efforts by them & their diabetes team to improve this haven't worked (or if despite loads of effort someone can't get their HbA1c below something like 8.5%).
It sounds like you'd qualify in terms of your hypos, and if you fit the guidelines no-one can refuse you a pump.
Ask for an earlier appointment with your hospital team & say you feel a pump would help & that you fit the NICE guidelines for one & see what they say. They might want to put a continuous glucose sensor on for a few days to look at your control (I did this), or they might suggest some changes you could make first, but if things don't improve, insist on a pump. Even if they fob you off, get in contact with Input (search on google), who will support an appeal by you, and you will definitely get one if you fit the guidelines.
