High Cholesterol, high insulin resistance, consultant says there is nothing else he can do

Interesting Tricia tried very similar foods last June with immediate effect felt great for a few weeks bloods did come down to around10/15 then July came still on blended food and just shakes started dramatically rising above 33 and haven’t really gone back down below 20 since then trod same diet about 3/4 weeks ago did not have any effect I’m only 12 stone I was only 11 stone when I was in my teens and playing football when I say I have rice poss once a week and not every week same as a backed potatoe poss 1 every month so not a lot of carbs I don’t also have bread as this spikes my bloods the most
 
Sorry kk123 just read yours typically wake up about 5/6 am test my sugars reading of anywhere between 20/25 I’ve tried breakfast of all kinds bloods do not like any food they will go up no matter what I have so I don’t eat anything no more I just have 1 meal a day at about 5/6pm just don’t get hungry anymore only eat because we have to exercise is limited now with problems
 
What meds are you on for the arthritis? Would there be any that could raise your insulin resistance?
 
I’m on a injection called enbrell take that once a week was on methotrexate but have stopped them now because of liver prob that that tablet gives you other than that just a statin and fenofibrate and dapaglfozin max dose some fish oil tabs and pain killers now and then would have a steroid injection which I no raises levels but I’ve never asked about the enbrell which I have every week which I have to have a grant for as this is an expensive injection I have not taken this a couple of times due to chest infections and I have immediate flare ups so I hope it’s nothing to do with that

I'm on Stelara biologic. I googled Enbrel and it's said not to raise insulin resistance. I'm also on Ozempic GLP-1, which is worth googling and asking your doctor about it reducing insulin resistance. Your SGLT2 could be switched to it, or both can be used with your insulin. AS you know the steroids would affect your BG and insulin resistance. I really would also look closely at my diet. If it were me, I'd look at a 20g of carbs a day diet

I'd look at the anti-inflammatory benefits of a low carb FODMAPS diet. I'd still base it around a 20g carbs a day keto diet.
 
OK. I don't know if your diabetes doctors know about it, but there is a National Severe Insulin Resistance Unit in Cambridge. It accepts referrals from all over the country for patients with unexplained severe insulin resistance. They cover diet, drugs, treatment and complications. If you Google it there should be a (fairly bland) webpage.

There are others like you, and there are still treatment options. I'm still severely insulin resistant, but my dose is down to 500u per day from around 900u. In my case, going on a pump and having continuous insulin, not injections, really reduced the insulin I needed for basal (about 50u per day now). The number of injections I was taking was creeping up to 50 per day, and is now down to one set injection every two days. Using cgm (libre2) means that when I am at blood glucose levels of 20 or higher I CAN keep on taking more and more insulin until the numbers finally move. If I'm going to crash hypo the libre alarm will tell me.

Thanks that is what I was hoping someone would give me a pointer I did ask this last year to the specialist he told me I had to of been a type one to be able to have this treatment he said in some cases they would do this .are you a type 2 diabetic? Thankyou
 
The rules about pump/cgm availability vary by nhs region, e.g. when I was applying Cheshire CCG would not put an adult on a pump, Manchester encouraged me to go on one. Type 2s don't automatically get considered, but you can argue that there will be a serious improvement in quality of life (reduced injections and fear of hypos). Your specialist sounds particularly unhelpful, which CCG are you?

I'm a type idiosyncratic, otherwise known as an awkward sod. I have had a detected autoimmune reaction, so I'm type 1. I had a slow increase in insulin needed, so I'm type LADA. I have severe insulin resistance, so I'm type 2. I have had genetic testing, and fortunately I don't have any of the hereditary forms.
 
Have you been tested to see if you could be type 1? It is possible to have double diabetes.

I definitely think Brettskee should get some more checks done in this area: the diet he outlined was a bit carby in places, but nothing that would suggest the kind of numbers he's posting, particularly after insulin. Perhaps it's not T1, but *something* seems to be awry, and I feel like all available checks would be a good idea.
