How to tell if med dosage too high?

I was just wondering, say someone was diagnosed with diabetes and prescribed metformin. They started taking it but also went lower carb, added in exercise, etc. So after some time, the changes they made with diet/exercise/other kicked in and maybe their dosage of metformin was now too high. How would they know if this was the case? Would they be low all the time? Would they have more frequent instances of hypoglycemia? Too low in the mornings upon waking?

This is just kind of a hypothetical situation I'm wondering about.

And would it be the same with berberine? Berberine is supposed to work similar to metformin, right?
 
In Canada, when you are first diagnosed, you will be monitored by the GP on a monthly basis to check how you are doing with the medication. In this way he can adjust the medication if it is too high or not.
