Ideas/Help with BG

Hello,

I've been having ongoing problems with my BG always being a bit high, and more recently it seems to be all over the place.

My BG in a morning tends to be high (12+) i have noticed that after having breakfast and taking both my background and fast acting (novorapid) 2 - 3hrs after my bg has barely changed. then all of a sudden it crashes and i either hypo or go borderline.
for example today:

Bg before bed last night = 6.6 took 10units levemir
Woke up (8am) bg= 14.4 took 10units levemir and 5units novo (30g breakfast + correction)
12.12pm bg=12.9 (30g lunch) took 5units novo
3.45pm bg=5.7 and shortly after I hypo'd

I've just looked on MIMS and found out that the onset of Levemir in approx. 2.5hrs ( i thought it was about an hr).
could the morning high be related to the slower onset of levemir?
I have always been on levemir and novorapid and they used to act much faster (eg. novorapid over and done with in 2hrs).

I've been to the diabetic clinic and they were a bit stumped. :roll:

Any suggestions?

At first sight it looks to me as if your night time levemir is a little bit low and your daytime levemir is maybe a little bit high.

Are you able to maybe do a carb free day to see what is happening to your basal? Sometimes that can give a clearer picture, when the carb/quick acting interactions are taken out of it.
I think if it was me I'd up the night time levemir a unit, have a look at what happened. When I got an a.m. reading I was reasonably happy with, then I'd either fast or avoid carbs for the day and test to see what was happening to bg (it shouldn't waver that much if the basal is right, but if you are still going hypo in the afternoon it would suggest that the basal's too high).

let me know if I've made no sense!
 
thats what i was thinking but i just wanted a second opinion! I am going to try and have a carb free day tomorrow to see what is happening. my only concern is that sometimes I wake up with perfectly acceptable BGs (eg.7) but thats not very often, and sometimes I hypo during the night ( but its usually an hr or two after i take my levemir and i suspect i am possibly taking too much novorapid with my tea). i'll have a go and see what happens! :D

Good luck, Let me know how it goes - i've been struggling with basal too lately, i THINK I've cracked it now but am waiting to see with fingers x'd.
 
The only way of finding out whether your basal is correctly set is the fasting test! Test every hour to see what is happening... Do remember if you go too high or hypo then you need to abort the fasting a take correct accordanly..

It's easier if you split your fasting test in sectors of the day, morning runing to just past dinner, overnight and then have brekkie, miss dinner and then do the afternoon can do it any order you want, over night basal testing can been done on one night if you fancy being disturb every hour, or you can do one night testing up every hour up to about 1am, then the following night start at 1am through to morning, or may be one night test every other hour and the next on the other hour you missed.. but remember for nigth testing have carbs that don't linger in the system such as pasta or a high fat content meal that slows adsorption.. And if you are doing testing over several night your bedtime BG should be pretty similar, and you ideally same sort of routine during the day ect..

Don't forget if your basal is out, you will need to reject your insulin to carb ratio's for the quick acting insulin as you may have been over or under correcting for the missing/over dose of basal insulin!
