Insulin and sugar levels

there are a lot of other factors to consider -- when you inject insulin you are taking over the function that non diabetics take for granted --- all of us D's have the addition of stress , and exercise , hormonal changes , and just general life that non D's bodies cope with for them.....................we must try to do the best we can as often as we can .............

Are you bolusing enough in advance of your meals @static192 ? That can help spikes a lot.

Also, just speaking personally, if I was 10, I'd correct and wait till I got down to around 7 before I ate.

Is there any particular meal that's causing you concern?

@azure taught me the invaluable lesson of pre blousing. It makes a HUGE difference. I eat 20 g or less carbs a day and I prebolus 20 min in the morning for just avocado and a thin turkey slice. Lunch and dinner are 20 min unless I want a slight drop. Then 15-20. It takes a bit of testing to find what works but ore bolusing does work and I would imagine very important tif eating carbs that get there faster than fat and protein. Since I eat very few carbs my protein gets their faster than if I did eat carbs

As @azure asked, is there a particular meal yore concerned with? I'm boring, ineat the same TYPE / MACRO meals at the sameness time of day so I know my timing and bolus based on where I'm at.

That could depend on the macronutrients in the meal. If high carb you might want to slowly back up your bolus. Waiting forminsulin to get there at the same time as your carbs. Getting insulin there at the same time as your food is magic. If insulin gets there first you will go low and if insulin gets there after food you will go high. You will need to do extensive testing to find the sweet spot.

How often does the insulin seem 'not to work'? If it's a frequent thing, you could try eating the same meal for a number of days - exactly the same - and do your bolus injection and note where you injected it. This might help you see if there's any absorption issues. The only time it seemed like my bolus 'wasnt working' was when I injected in a particular spot on my tummy (when on pens, I have a pump now) and so I learnt to avoid that spot.

If it's an infrequent thing, then think about whether it might be to do with the insulin itself - eg is your pen old, was it stored correctly, etc?

That could be lipo issues, ask your DSN or Consultant to check your injection sites next time you see them, but to reduce postprandial spikes bolusing in advance (as @azure mentioned) is a must to give the insulin a head start over your food digesting.

Hang on a minute! there's no mention of amount of carbs in your notes, that's a vital bit of information missing.

Can I suggest you write notes out with date and time of each injection or test. For meals note total carbs, and what it was - if you want you can break that down to each component of the meal, your ratio you used, correction if any used and why, and any notes that might help like time of pre-bolus - as an example:

22/12/16 22:50 BS 13.0, 6u correction (for 6mmol reduction), 22u levemir.
23/12/16 07:20 BS 7.7, crunchy nut cornflakes (70g) with milk = 50g carb, 1:5, 10u, +1 correction (for 1mmol) Total 11u pre-bolused at 07:10
23/12/16 07:45 levemir 18u
23/12/16 09:20 BS 11.1 (probably need to pre-bolus earlier then!)

It is then very clear what you have done and what you could change or tweak.

I agree with @slip The more information you can give, the easier it is to make suggestions
