I get this occasionally and will become aware to look for it when my blood sugars rise dramatically. It means the insulin isn't going into me any more.
Trouble I've found recently is by using my legs I can't always see the plastic of the cannula window. I've tried taking a picture of it but that didn't work. In the end I had to ask someone to look at it for me, not ideal.
If it happens on day one it normally means the cannula kinked going in.
But it can happen on the second or third day.
I do regularly alternate my sites and am careful not to have too big a dose going in at any one time
