It's been one week. 5 units before each meal and 10 at night. 4 shots a day and I have had no improvement. Never a reading below 250 and high as 487 (USA citizen here). What should I do? Not scheduled to see Dr. for 2 months.

Ok..do you know how many grams carbohydrates you did eat on each meal or could you give us example of your typical meal (or daily diet) on your plate? It is quite likely that that the insulin is working, though maybe not efficiently enough and you need to tweak your energy intake and/or insulin levels.
Do give us more info so we can help you and point you to right direction.

In addition to what Finsky asked, can you tell us if you're type 1 or type 2? Can you also tell us what type of insulin(s) you're taking as well?

It sounds like you're on set doses of insulin which rarely yields good control. I'm willing to bet you were also told to eat a certain number of carbs each meal (it's called the "Sliding Scale"). There are certainly ways to improve on that (carb counting), but let's not get ahead of ourselves.

I'm in the US myself and use Levemir (long-acting/basal insulin) and Novolog (short-acting/bolus insulin; they call it "Novorapid" in the UK).
