is this a very large dose of insulin to be on??

Hi everyone,
still very much a newbie with my questions, and here is another one, hoping as before, you can help

Ive started insulin 4 weeks ago, and am on levemir one a day injection, and the clinic have been upping my dose regularly, am now on 48 units, once a day and Im still not in the ideal range

Fasting 8.3 before lunch 10.3 before dinner 9.2 bedtime 11

I am watching my diet carefully, and eating sensible balanced meals low gi

I think may be next week my DSN will increase my dose again to maybe a split dose,
I just wondered reading about so many on much lower doses than me whether I am going to be ok on just Levemir or whether its likely I will go on a different regime ie basal bolus

Has anyone had anything similar?

Would very much appreciate some help

Are you testing 2 hour post meals to see what numbers you are reaching then? I Forgot are you LADA?
Or Type 2 ?
It does look like a large shot of Levemir in one go :think: Go with your DSN and see if a split dose will help, but I'm sure the basal / Bolus will be just around the corner. Test after meals and go armed with a few days of those readings to show your DSN.

my post meals readings are not too bad, goes up two or three points, ie before dinner yesterday was 9.2 2 hrs later was 10.0 just seems across the board high

I was hitting the high 20,s though just 5 weeks ago up and down, my beta cells just seemed to give up all at once, my prev Hba1c was 7.5 just on metformin and the next was 9.5% so they tried glicazide to no effect. and I went onto insulin I must have some insulin resistance to need this amount of insulin surely They are still not sure if i am 1.5 or type two... they are going to do a blood test when i go back

I do need to lose 2 stone but not obese at all

Shall take all my readings in with me next week I have hit a few 5.5 and 6.2 ect but think that is as im really trying hard with my calorie intake and doing lots of walking and swimming to help myself as much as i can

As with most replies, if you have any concerns,contact your doctor or diabetic specialist nurse. Every ones requirements are different. I need a daily dose of 70 units of levemir along with 3 doses of fast acting novorapid to keep my sugars under control. Levemir I believe is a one a day injection. Your DSN may recommend you start on something like novorapid which you inject before each of your 3 main meals, i.e breakfast , lunch and evening meal. After testing your blood sugar before you eat, you can give an accurate dose of insulin. This system of injecting, known as basal bolus has had my HbA1c down at 5.9-6.6 during the six or more years I have used it. My average before that using the old two injections a day system was 10.5 which is way too high.

Well it looks like they are certainly moving in the right direction...No great spikes after meals but you don't want to be killing yourself to get good post meal readings, if a bit of fast acting will do the trick.
You can be Type 1 and be insulin resistant too...Type1 ,Type2 ,LADA ,MODY ..YADA YADA YA :wink: Does it matter so long as we get decent control and find the best ways to do it ,whether it be diet exercise, insulin, meds.OR a mixture of the lot :lol:
Maybe they don't want to rush you into Basal/bolus if you are still producing a fair amount of your own insulin...a world of Hypo's is no fun.
At least you are moving in the right direction Ali, I'm sure it will all fall into place pretty soon and you are on the right track and getting back under control...it does not matter how you do it and it may take a bit of time, we are all different but just think about the gains you have made so far.
If you keep on with the diet and exercise as you have been doing and get rid of the 2 stone you will really find that the insulin resistance improves..so well done on all the lifestyle changes. This applies to whether you are on insulin or not...Keep up the good work Hun
 
thanks so much for all the advice, it is such a relief to me that i found this forum at the very time i really needed it, you guys have made my first weeks bearable, and ive learrned so much
