I've just been emailed a link to NHS "Know Diabetes" app to manage T2D.

A very welcome development. An app called "Know Diabetes" with a dashboard including "My Results", "My Profile", "Managing your Condition", showing graphics of results and screenings, to help us manage T2D. It's incomplete, less detailed than it could be, but hugely helpful, especially as I didn't keep records of my HbA1c and lipid results over the years. Is this just new to me, or is the NHS finally seeking to tackle the epidemic of Type 2 diabetes by involving and informing patients?
 
Not heard of this one but I joined my diabetes my way a few years ago and it has all the info you mention. It's great being able to check my results, seeing them in an easy to read chart and I've linked my fitbit to it so can track that progress too. I think its managed by NHS Tayside and was told about it by my practice nurse.
 
Lucky you. I guess GP surgeries are very uneven on help for T2 diabetes. But my practice is beginning to come to attention recently, maybe because I've had quite a few problems, although I sometimes feel like a nuisance.
 
Is that app only for NHS England ? We don't usually get that sort of useful help up here in Scotland.
I haven't looked at it thoroughly yet, but I see the app is regional (NW London) and with a 2025 copyright. @AnnHaugh has had a similar app for some time, so I suspect regions are coming to realise the importance of self-help. Having said which, it's far from comprehensive and has the usual advice for Type 2s, essentially to "take your meds, eat a balanced diet and avoid sugary foods". You should ask your DN or GP about it.

For Scottland Only:

you need to register i've linked the registration page, sign in with gov gateway. they then will send out a form that requires you to sign and send back to them (this can take a little while). after agreeing to terms and conditions on the form which is sent out (to allow them to process the data) you will then be able to view your lab results amongst some other other fairly useful data.

there is some info/courses mentioned which require you to sign up for https://mydiabetesmyway.scot.nhs.uk/know-more/my-lifestyle/elearning/ you have access to those as soon as register and login you will be able to view at least those parts without waiting on having to send them the form back :)

you can register at https://mydiabetesmyway.scot.nhs.uk/.
With this I am able to see my test results as soon as they are done, no need to call surgery.
You can also see earlier results as they develop over time
Measurements include HbA1c, detailed cholesterol - HDL, LDL, Trig,
weight, blood pressure and more. I find this very useful.
 
Looks like it’s in beta for certain areas in England so depending where you live @Rosie9876 may be why you got the email.
