Type 2's are insulin resistant, gradually worsening over time (esp. if not well controlled and lifestyle changes made). Their primary problem is hyperglycaemia. As the resistance gets worse, the medication starts and then increases to the maximum allowed. Their insulin production also tends to reduce. Once that point has been reached, the next option is artificial insulin.
However, the type 2 is still insulin resistant, so pills are often needed in addition to the insulin to combat that aspect. The lantus, therefore, is given to supplement their natural supply, which is no longer sufficient for their body's needs.
