Lantus insulin split



I am considering increasing my evening lantus to get lower numbers in the morning.



I read that some type 2 diabetics split the dose, half at night and half in the morning.



How does that help? What are the benefits?

Many people find that lantus only works for about 20 hours, giving them a few hours of higher numbers or needing more bolus. Also, if you tend to go low in the night you could take the bigger part in the morning and if you tend to go low in the day you could take the bigger part in the evening.
I was only on lantus for a short time so I haven't been experimenting with split doses but someone who knows more about it than I do will come along :)

The idea is to be able to take different doses for night and day as well as to ensure it actually lasts 24 hours without a gap in basal coverage. It also allows you to adjust your basal dose every 12 hours according to your needs (a little bit more like a pump). For example, I take slightly less in the morning on weekends vs. during the week as I am more active, less stressed and therefore need less basal or will be going low all day. I take more at night when I eat a big dinner or dinner high in fat/protein as it helps with the spike I will get in the middle of the night from delayed digestion. Personally I think Levemir is a better insulin for splitting than Lantus, as the duration is almost exactly 12 hours at small doses for most people, so there will be no overlap. It is also more responsive to changes in dosage, more consistent from day to day, its consistency/effect is less dependent upon the injection site and it is more stable/lasts longer even once a cartridge/vial has been opened.
