hi, i am type 1 and my bg levels have been horrendus for the past 6 months. if they are as low as 13 i think that is an achievement!!!!!
i an currently taking 27units of lantus at 5 in the afternoon, if i take any more then that i hypo in the mornings, i am dropping about 5 or six over night on the regime i am on.
seeing as i am then running high all day, i think the lantus would be more effective if i took it in the morning instead. so how do i go about changing the times. do i take 13 units at 5 oclock today and then 14 tomorrow, followed by 27 every morning from then on, or 13 units this afternoon and 27 in the morning.
i have another month before i see the DN and cant stand thesse stupid high levels anymore.

Your question finally prompted me to sign up - I'm usually a watcher not a 'talker' lol
Basically I was having the same issues as you with my Lantus and hypos and highs. I could never stabilise. Its quite obvious really isnt it to think to take your Lantus during the day when it has most work to do!
Anyway, in answer to your question, it really depends on how confident you are making adjustments to your fast acting insulin (I use novorapid)
I just took my Lantus a few hourse earlier every day for about a week, but I reduced my 22 units to 18 to avoid any nasty hypos and just lived with the higher BG's, adjusting the fast acting to suit. I did bounce around a bit.
Once I'd got to the right time for taking the Lantus, I did BG's 4 -5 times a day to check for unwanted highs and lows and made adjustments to up my Lantus to 21 eventually.
Since then my HBA1C has gone down to the lowest its been for about 15 years! And I feel so much more in control.
When I initially suggested it to my DN, she said not to do it, that I was just becoming less responsive to the insulin and to take more!! Pfft :crazy:

First of all I'd advise you to ring your diabetes clinic and ask to speak with your DN on this matter with your next appointment being a month away, most are quite happy to speak over the phone or will return your call should they be unavailable

Looking at what you've wrote and seeing that your bg drops by around '5 or six overnight' and you hypo in the morning I would say your lantus dose is way too high and may need reduced, as for your double figures during the day.........are you sure your highs aren't a result of you over-correcting your morning hypo's or a case of giving a incorrect insulin ratio to the food you eat?

Were I to move my basal dose from 5pm to a morning then I'd move it forward gradually and inject the dose at 10-11pm for a few nights to see if this helps, if it doesn't then you could change to a morning dose but the time difference between 5pm to 11pm will make this change all the more easier. Just remember to test frequently (especially during the night) when adjusting doses and timing of doses with lantus and allow at least 3 days for the changes to take affect.
