lantus v levimir

Hi all, just wanted to seek some opions lantus against levimir. My daughter (14) is currently on 19 units levimir at night and carb counts and uses varying doses of nova rapid during day. the problem is that now late afternoon in to the evening her levels are starting to creap up and the smallest snack leads to quite a dramtic rise, It has beeb suggested this may be due to her levimr not covering her for the whole 24 hours which we were told was a possibilty when she was diagnosed and were expecting to end up having 2 doses of levimir a day.

However her dietician today mentioned switching to lantus as a possibity as will cover last much longer, just wondering if anyone else out there has made this switch or has any other expereince with lantus

My son (4) has always been on Levemir but the dose is split morning and night. I've never had him on a once a day dose. There are quite a few questions about Lantus on this forum and other places where people have found it has affected their health. There are also many numbers of people who have no problems at all with Lantus.

and here is a graph comparing the profiles of these basal insulins -- where Detemir = Levemir, Glargine = Lantus... as you can see: they might be advertised as flat/peak-less but this may not be the case, although both are better in this respect than NPH

If you do switch insulins: because of the different mechanisms for their slow release (as described in the link above) you may find that it is not a straight swap, dose for dose.
 
Maybe go down the route of splitting the dose first if she is running out by the afternoon, say 30% on a morning and see how you go with that before going on a whole new insulin which will jig her overall control, because if that is good with few Hypo's I would not want to mess too much if it was me.
If that does not heed good results then go with what the nurse says and give Lantus a go but you may need to start adjusting your Bolus doses on the Lantus.

Hi Shelly!. I used to take lantus,but have just switched to an insulin pump. On lantus I needed to split the dose, so I would suggest your daughter tries this with her levamir rather than switching insulin and see how that goes to begin with. I have frequently heard that long acting insulin does not last a full 24 hours. best of luck xxx
 
Hi, just wanted to add since I split dosed my levemir insulin 2hrs after shot feel more perkier apparantly it does take 2hrs to reach full onset. Giving better coverage as split dosed for me than single dosed.
