Libre Sensor 2 plus sensor

Has anybody had a sensor fall apart or looks like 2 sensors together.
See picture attached
This has just happened to me but I can't praise Abott enough when I complained.
Sending me a new one as a priority.

Hi,

It’s not clear on the picture.
However, there has been the odd case where the hole punch from the applicator that buries the filament has not retracted & broke off.. which is what I can make out from your images..?

Are you suggesting there were 2 sensors stacked in the applicator?
 
Like Jaylee has said it looks like the punch has not aligned properly and instead of penetrating your skin it has pushed back and broken the sensor apart , I have had it happen once , not as dramatic as breaking the sensor apart but pushing the punch and filament to a angle , I just phoned Abbot and they replaced it .
 
Hi not sure but when applied sensor it seemed like sensor attached as normal but then I saw in the applicator another bit was left with a small screw sticking out and another needle.
But would not scan.
Abott we're replacing.
This is the second one that failed in succession the other one said replace sensor everytime I scanned but was still giving reading.
 
I have heard cases of the “hole punch” being left in after application, it’s meant to retract but somtimes breaks?
. @urbanracer had it happen?
