If you've had the training and feel confident I wouldn't wait for your DSN to start setting up your basal rate from a flat profile. Slowly and gradually is the key though, especially as you are coming from having a high HbA1c. You can experiment first if you like by using temp basal up or down by 5-10% or go straight into adjusting the basal settings. Don't forget to keep a diary of where and how much you have adjusted your basal so you can see how your doing. Also don't forget that your basal will have a lag as a rough guide it's 1-2 hrs, for example if you want your bg to be lower between 2-6 pm you would increase your basal from 12-1pm to 4-5pm. I would slowly bring the basal up and leave it a few days between each tweak, take chunks of the day to tweak at a time then move on to the next chunk. Let us know if you get stuck with anything.
