Low blood sugars

I had a gastric bypass in 2018 to try to reverse my type 2 diabetes. Unfortunately although I lost weight to normal BMI I still needed insulin. After complications in 2019 I had some of my small bowel removed but still needed insulin. My latest HBA1C was 6.5 (46). I am on 20 units Levemir ( 14&6) and 4 units of Trurapi every meal. My question can someone's diabetes suddenly change without me doing anything differently with food. I am experiencing more hypo's. Had anyone else got better control without trying. All other blood tests results are good.
 
are you doing more exercise than you used to (inclusive of just generally walking about more than before)? :) exercise can influlence sensitivity to insulin. congrats on the weight loss :)

are you using a cgm with your basal and bolus routine?
 
I have increased my exercise but this is a walking work out video. I have severe osteoporosis so no heavy lifting or running. But I don't think this is why suddenly my HBA1C has dropped as I have been doing these exercises for the past year. I use finger testing.
 
Is there a particular time of day when lows are happening?

insulin requirements can change over time. It would be worth speaking to your diabetic team. have you done any educational courses such as https://dafne.nhs.uk/victor/ ? if not would speak to your dsn and ask for a referral, if have and was years ago maybe they can arrange a refresher course. Whilst at above would request if cgm prescription is perhaps possible even for a short term due to hypos. It would be very useful to work out any potential patterns. There is a couple parts within nice guidance that might help to be prescribed

If you haven't have cgm trial dexcom is available at: https://dexcom.com/en-GB/dexcom-one-plus-sample (10 days) and abbotts do a 15 day trial at: https://www.freestyle.abbott/uk-en/getting-started/sampling.html (be aware with both cgm's lows and highs should be checked against fingerprick too). might be able to work out a pattern which in turn could help with dosage (via dsn). (using both trials 25 days of data to look at)

as you've said same dosage with all meals 4 units of Trurapi are all meals around the same carbohyrdate count?

referred to as 'flash') to adults with type 2 diabetes on multiple daily insulin
injections if any of the following apply:
• they have recurrent hypoglycaemia or severe hypoglycaemia
• they have impaired hypoglycaemia awareness
• they have a condition or disability (including a learning disability or cognitive
impairment) that means they cannot self-monitor their blood glucose by
capillary blood glucose monitoring but could use an isCGM device (or have it
scanned for them)
• they would otherwise be advised to self-measure at least 8 times a day.
