New Basal Bolus Regime

Well the time has come and just wanted to say that my son James (13) has today started on his new regime. Started off with a bg of 17.2 this morning (this reading has not been unusual for a few weeks in the morning), hence going onto multiple injections now. Breakfast morning all worked out, gave his units plus correction dose and was 17.1 when he came back from football at 12 noon. Dad said he was not so energetic on the field. So again gave units for lunch plus correction dose and when he did his bg 2 hours after lunch he was down to 4.4 and shaking which we have now treated as a hypo. Taking tea into account and supper, I have then got to give him 16 units of Lantus before bed.

I was told that it will be a (false day) today as he did not get any Lantus last night before bed, so I guess tomorrow will be the day to see how his bg's will be.

You should start to see different bg levels tomorrow I would think, but getting back to tonight when your son has his first Lantus dose, be careful and get up about 2am to test his bg. If the amount of Lantus is not correct, it will make bg levels drop or be elevated.

Lots of people do get hypos around 2am and by testing you will be able to pick up on them and eat some carb before they get serious. Everything will be 'trial and error' for a while until you find yr correct dose. I always eat a small amount of carb before going to bed with Lantus to prevent hypos.

Don't be surprised if you need to reduce the Lantus and then find that after a few months, you need to reduce it again and then.... you may find that you need to start increasing it after about 12 months.
 
good luck its daunting starting a new regime we have just changes to the pump and its all scary but soon get going we found a big difference going from the mixed insulins to basal bolus hope it helps ur son :D
