OMG - we forgot Lantus

:shock: I got a bit panicky this morning as I had only just realised that James did not do his 16 units of Lantus before bed.

I immediately woke him at 7am to check his BG and it was 20.9. I then got him to do his 16 Lantus and then 4 Novorapid and when he got up again at 9am his reading was 14.3. He then did a correction dose plus his dose for breakfast now we are waiting to see what happens next.

What worries me now is cos we did the Lantus in the morning, will there be a problem with doing it again at 10p.m. tonight.

I will be emailing the DSN straightaway but wonder if anyone has done this and can give me some peace of mind.

It happens! When we were using MDI I had my mobile phone alarm set to go off every night for the Levemir injection otherwise I'd forget. We used to split the dose, the breakfast one we didn't forget because she had it with breakfast but the evening one wasn't with a meal and we'd forget. Perhaps if you do something similar, have an alarm set, it will help avoid this problem.

I'm not sure about Lantus, we used to use Levemir. But I believe if you missed the night time you could split it fairly evenly in half and give that half the next morning. But keep a close eye on readings for the rest of the day. Correct any highs with Novorapid and any lows with the usual hypo treatment.

As you've done the full injection this morning you'll need to speak to your DSN as tonight is going to be tricky as to how much to inject. If you do the full injection then he may run the risk of going hypo during the night. She may suggest missing an injection again tonight, then tomorrow morning doing half the dose followed by the full dose that evening.

I had a similar problem recently, with Lantus. I just returned to normal timings, but reduced the dose by about 4 units (normal dose for me is 18 so I injected 14). I was a bit high next day but nothing horrendous. I think it's better to slowly ease back than get into one of those high/low flipflopping situations which just make you feel ill.

After the horrendous 7.13am early morning reading of 20.9 his readings have been

9.09 14.3
12.15 7.6
14.40 8.9
16.41 3.2 (treated hypo)
17.19 9.4
Just tested now and was 14.7 but I reduced the insulin before tea by 1 unit as he is now off to football training and back at 9pm

After emailing the DSN about forgetting the Lantus last night she said "it is advisable to reduce the lantus dose to 12 tonight as you will have an overlap.You may then find he will be a bit higher tomorrow and the morning but at least we will know the reason.We will have to rectify by doing correction doses.

I will be back tomorrow to let you know the outcome.

Hi Again

The day not as bad as I thought after forgetting the evening Lantus etc. Gave 12 units as DSN suggested and James woke at 8.13 am today with a BG of 4.7 (Phew). The rest of the day was as follows

11.07 - 10.4
12.29 - 6.4
Went Swimming in afternoon
16.16 - 4.0 (treated hypo)
17.00 - 8.5 (off to football training)
Just back home with a BG of 3.9 (treated hypo - but did not feel he was low))

I am waiting to see now if DSN says he can go back to 16 Lantus tonight and we will be seeing her in clinic tomorrow afternoon with all his readings.

Will be back again tomorrow evening with any news

I forgot to take my Lantus one evening (normally 10pm)... only remembered the next day at about 2pm!

I considered taking a reduced dose (half) and then carry on like normal, but I decided not to. I returned to my usual 10pm time slot that day.

My blood sugar was a little higher than normal but not too bad - I found I was using corrections at every meal.

I spoke to my DSN and she said because the effects of Lantus are spread over 3 days (that's how long you have to wait when changing dose) it's not as bad missing one lantus injection, as opposed to one levimir injection.

She also said that taking an injection when I remembered would have been a good idea (albeit reduced). She compared this to when you travel through timezones, sometimes you have to move the injection by more than 30mins...

Glad it worked out ok!
