personal care plan

as i,m type2 and have depression, my gp surgery wrote to me asking if i want to have a personal care plan done with my gp
it sounds like a good idea and thinking it would be a good idea
has any other members surgery doing this?

Go for it. It is one of the 15 essentials that DUK say are the minimum we should expect.

9. Receive care planning to meet your individual needs. You live with diabetes every day so you should have a say in every aspect of your care. Your yearly care plan should be agreed as a result of a discussion between you and your diabetes healthcare team, where you talk about your individual needs and set targets.*

Make sure you have a list of questions you would like to ask as your involvement is essential If you do not understand what is being said then say so as some of us do not understand all the possible jargon that is often used.
 
I'd go for it, even though i'm not actually sure whats involved.

Even email contact is good, I've found that useful with my DN, saves traipsing to the surgery all the time.

They'll plan review appointments as well i assume so you can get them in advance.

Plus hopefully its a nice doctor so good to offload a bit as well.
 
I was given a 'personal care plan' last week, 4 years after diagnosis. In my case it was just a booklet, it had spaces to be filled in with HbA1C, blood pressure, weight, BMI, foot check, cholesterol etc.
At the back of booklet it had a red/green type charts showing what the current HNS guideline/targets are for HbA1C, cholesterol BMI etc.

For me it wasn't much use - would have been really valuable 4 years ago!

However - I always think its worth getting an MOT/personal plan from GP as often as you can (as its not offered very often!) Your GP might offer a more tailored personal plan.

You could be a forum guineapig, go find out what you're offered and report back
 
Yes I think it's a great idea Ally. I would accept all the help your surgery can offer you. That's what I usually do, I only look elsewhere after first seeing what my GP can offer me. If you don't check out what they are offering you won't know if it's any use to you or not.
 
i wanted some opinions and got the result i wanted. i did ring the surgery to say i would go ahead with it, i,ve no idea how long i wuill have to wait until i see my gp

I think with the struggle you've had to get any support at all, from your medics, it's great to see something coming your way at last. When you see your GP, please ask him to prescribe your testing supplies. I don't believe they can prescribe for the Codefree, but if they will prescribe at all, it would be worth changing meters.

Oh, and did you ask about the exemption form? As I understand it, once you have applied, if you keep any relevant receipts for prescriptions, you will get those fees back. That last bit is something I think I read on here. As I don't take any meds, I don't qualify for free scripts.

I don't suppose your reference document suggests how much the NHS pays for the strips? Just out of interest.
 
I received a letter this week from my GP surgery saying they are bringing in compulsory care plans for all diabetics registered there. They suggested brining in a list of questions you may have and if you have specific things you'd like to discuss. It will be a 30 min appointment where you'll agree goals etc and they will review them regularly.
I think it's a good idea, have mine next week.

My Practice already follows the Care Plan guidelines. I was put on it in January when I was first diagnosed, although I only got my record book at my last visit in June.
 


Ooooh, excellent. I wonder if my practise will do that too.

Where I agree and objective, I will expect them to agree to an objective too. I may have to think about that a bit before I decide what I'd like them to agree to. Objectives are brilliant. I bet you lot are glad you're not managed by me.

andbreathe, stupdily i forgot to ask about the exemption form
 
i,ve just rang my surgery about the exemption form, they put me through to the pharmacy
they told me to go into the pharmacy to fill in the form, they will get my gp to sign it and they will send it off
 
I wish mine had offered one. Or even a diabetic nurse appointment. Ours retired just as I was diagnosed so I have seen the nursing assistant who is picking up the slack. So far I know to moisturise my feet and wear slippers at all times and have had my routine blood test taken a month early as I was " already there." Now I have to phone GP as no one has sorted a repeat med prescription for me as it can't be done without a review, which I haven't had as there's no diabetic nurse and the nursing assistant probably shouldn't have done the blood test early. I feel like I am going round in circles sometimes. No wonder I feel stressed.
 
2% of patients at GP's have to have personal care plans. Our GP's identified that the elderly in residential homes were priories but, other patients would be considered if they requested them.
