I received a letter this week from my GP surgery saying they are bringing in compulsory care plans for all diabetics registered there. They suggested brining in a list of questions you may have and if you have specific things you'd like to discuss. It will be a 30 min appointment where you'll agree goals etc and they will review them regularly.
I think it's a good idea, have mine next week.

I haven't had anything.My Practice already follows the Care Plan guidelines. I was put on it in January when I was first diagnosed, although I only got my record book at my last visit in June.
