Pre-op assessments

I have a collection of dodgy joints awaiting one repair or other, and recently attended a pre-op assessment clinic at my local hospital. There, I went through a series of tests, including an HbA1C, which returned a result of 9.8 in old money. Consequently, I was bumped from the list, and referred to the hospital DSN for advice on reducing the A1C to below 9.

I was at the time, taking standard Metformin (500 x 3 daily) and Sitagliptin (Januvia). I can't tolerate Metformin in any higher dose, as it causes vomiting. The DSN, after stating the obvious about me being overweight, (correct, but not hugely so), said that it is normal practice to start any Type 2 with an A1C over 8 on Insulin. I am not keen on going down that route just yet, so agreed to try Glucophage SR, which is the generic Metformin, at 2000mg daily. Strangely, if I take this after my evening meal, all kinds of digestive Hell breaks loose, but taken after lunch, all is (relatively) well.

I am now testing on a daily basis, and the numbers are coming down nicely. BUT, I am slowly but surely putting on weight, which is a deal-breaker for me. Has anyone else experienced weight gain with the SR formulation? I feel I'm in a Catch-22 insofar as I must get the A1C down in order to have elective surgery, and I must have the surgery in order to become more mobile. I've had conflicting advice from my GP and the DSN, and am now completely confused :? Should I now review my diet to mitigate the effects of the Glucophage?

My next appointment is on 3rd April, by which time I hope the A1C will be well within the required range. I just hope my weight doesn't balloon in the meantime!
