Hi and welcome to the forum.
I do not have the said phone but have read on line that some people have had blue tooth problems when using it in combination with Libre sensors they found that often they would have to restart the app inorder to reastablish their connection this probably may not apply to your particular problem but I hope it maybe of some help.
