I do think you need to go back to the doc, ask him to adjust you vit D and ask him for an Hba1c. Say to him that something feels like its going wrong and you've decided it's time to review your lifestyle and make changes to improve your health - usually the doc will respond more positively to someone who comes across as wanting to take control of their situation.
