Really worried about levels

Hi everyone,

Been type 1 for 18 months now and I've always had good control. But for the last couple of weeks my control has be poor and I have started getting highs as much as 20.

The worst thing is my morning bs. I can got to bed with levels of 7 and wake up with levels of 17

One day I didn't eat for 9 hours But still had levels of 16 before eating again.

I'm starting to get really worried about this and was wondering if anyone can suggest something I can do to lower these readings or a reason for these levels.

Hi Mark,
Sounds like you're coming out of your Honeymoon period to me. This is when your pancreas produces insulin for a while before finally giving up. It happens to most people after being diagnosed. Might be useful to speak to your DSN.

I think you need to increase your Lantus. Test before bed and upon waking. For every 1.6 change in BG levels adjust your Lantus by 10%.

I recommend buying Gary Scheiner's Think Like A Pancreas book. There's loads of useful information in there.
 
Agree with Sam, it all points to your basal insulin and you need to increase the dose, have a word with your dsn who will advise you further.

I cant get in with my DSN for another week. so in the meantime would you suggest i up my Lantus and my insulin to carb ratio?
 
I reckon so. What were your bedtime and morning levels last night/this morning? When your insulin requirements change its best to get your Lantus correct first because you'll be chasing your tail trying to do both at the same time.
 
Agree with Sam. Basal needs changing. Can't you call/talk to someone from your care team before your next appt? Once the dose is changed it'll take a couple of days to see the effect so don't think nothing is happening and increase further.

my levels last night were 8.2 at 11pm, this morning i was 13.5 at 7am. I increased my Lantus from 12 to 16.

I have only just moved to my current practice, so im not really in the system with them yet. so i have no numbers or names.
the only people i have on call are two blood uncles who are both type 1 for 30 years.
 
Did you increase your Lantus last night, or are you planning on increasing to 16 tonight? Assuming that youthe had 16 units lastto night and using the formula I gave above your Lantus dose needs increasing to about 20:

(13.5-8.2) = 5.3

For every 1.6 change in overnight BG levels you needs to change by 10%:

5.3 / 1.6 = 3.3
3.3 x 10 = 33

So your Lantus needs increasing by about 33%:

1.33 x 16 = 20.8

Up to 20 units. I personally would change by 4 units, others may exercise caution and change by 2 at a time. Feel free to post tonight's and tomorrow morning's results up.
 
sounds like your basal dose is too low, or you have the wrong carb to insulin ratio, or both. theres no need to worry, high blood sugars are usually caused by simple errors, however you should not let dangerously high blood sugar levels continue for as long as a 'few weeks'
 
Morning guys, again thanks for the replies.

Last night I was 6.8 at midnight, and then 11.2 at 7 am this morning.

My normal rapid is 1 unit per 10g. But I have seen this was not working anymore. At dinner last night i was 6.9. I had 140g of Carb, But injected 20 units.

I'm worried I'm changing too much at the minute
