In an effort not to be alarmist, I'll start by mentioning that Fournier's Gangrene has only been observed in 12 diabetic patients between 2013 and 2018. However, the FDA are issuing warnings about this class of drugs.

I was in 2 minds about posting, having thought "what are the chances", this is never going to affect anybody on this forum, however, just in case.

You present a balanced view point about a nasty but rare side effect. All drugs have side effects. This class works by getting the body to excrete additional glucose via the kidneys hence this particular effect.
Other drugs push the same glucose out into the fat cells or other tissues in tbe body and if patients do not modify their carb rich diets, the effect of that glucose is to rot/necrotise other parts in the long run hence the complicatons that occur even if people have good HBA1cs on those drugs but don't modify their lifestyle. That's according to Jason Fung anyway and he rates diet as the best cure but comes down on the 'flozins as better than their predecessors.
