Starting Dose - Questions.

Hi Folks,

Just started on insulin last tuesday. Basal/Bolus of Lantus/ Apidra. Docs started me at 10 units of Lantus and 2UApidra per 10 G of carbs at meals. using 8mm needles.

They stopped Liraglutide and Glicazide at the same time.

Well - what a rollercoaster - I have shot up to an average of 17.3 - yes average of 17.3.

Going back on tuesday for a dose adjustment, and I'm expecting a big one! lol.

I am not the smallest of chaps, and looking at the Lantus website here www.lantus.com/hcp/dosing.aspx it looks like the starting does the docs have reccomended is much too low for me.

Anyone else similar?

Richy

No idea about exact doses for those insulins, but I'd assume the procedure at the start is the same for all - start you off on a deliberately low dose, then increase it gradually, to avoid hypos at the start.

The first day, the tablets might overlap with the insulin too, which is another reason to keep the insulin dose low at the start.

On Tuesday, make sure they either tell you how to adjust the dose and how to do correction doses, or at least tell you when they are going to tell you if they want you to wait another week or so.

The webpage you have quoted says recommended starting dose 10 units per day (or 0.2 units / kg) for type 2 diabetes, followed by adjustment. It also notes that for people with type 1 diabetes, the ratio is 1/3 of total daily insulin as Lantus, which leaves 2/3 in short acting. What it doesn't mention, but probably assumes health professionals know, is that many people with type 2 diabetes take only Lantus. However, you have said that you are also taking Apidra, so that would mean your Lantus dose will be lower than if you were taking only Lantus.
Anyway, it does make sense to start low and adjust gradually.
