On reading your first line, I thought that, there are some endos out there that know about hypoglycaemia!
But because of your readings and observations, you don't seem to be having actual hypos.
The 11a.m. readings you are asking about is interesting for me, because that would be around the time that I would go hypo, when I was eating porridge for my breakfast. Depending on what you ate and the timing could be a delayed trigger for .many reasons. But because of your other conditions, and I presume the meds you are taking may skew symptoms and readings. Obviously I don't know, but noting down what you eat and the readings before and after, like you have been doing! This will give your doctors an idea why.
