A couple of times recently I have had the scenario of 0 units (RAI) required for a meal. BG 5.0 with a target BG of 8.0 and a 25 Carb meal (1 unit per 10g Ratio). However, my 2 hr blood has been very high with no IOB and I've had to take some RAI to correct.
I was just wondering what different strategies others may be using in this situation.
 
Are you using a meter to calculate that 0 dose? I only consider IOB for,correction doses, so personally I'd look at the meal carbs and inject for them unless I'd had a recent correction.

I always test after meals to make sure all's ok.
 
I'm using RapidCalc, so I end up with a meal dose and a negative correction dose (5 minus 8) which leaves me with a 0 RAI dose.
I too test 2hr after a meal hence my question.

When similar circumstances arise and your starting BS is around the same level, you could cautiously try having a small bolus to see if that stops you going too high.

As I said, I don't use bolus calculators (and never have done) but, in general I'd only eat around 10-15g carb without including it in the bolus. If I'm lower than desired before eating, I have a small amount of glucose if necessary and then just bolus closer to the meal.

I know that's not much of an answer, but I've found that, as with most things, it's trial and error.
 
Cheers, that's pretty much what I've been doing, just wondered if there was a magic rule out there somewhere.
 
are you pumping or MDI? If pumping I would take the suggested bolus for the food over a few hours (2-3) if your checking at 2 hours then you could cancel remaining if you dont need it, and it can cover protein/fat you've eaten too.
 
I think I know the answer to my question but why have your target so high?
 
My last Hba1c was 5.9%, which I was told was too tight (risking hypo-unawareness) though I rarely have hypos, so I upped my target from 6.5 to 8 in an attempt to get my Hba1c up to 6.5%.
 
Ah, if only there was a magic rule! If in doubt, I proceed cautiously and add insulin or food as needed.

My last HbA1C was 5.2. I aim to stay between 4 to 7.8 as much as possible (but don't always achieve it) If you don't have a pump, you may find a half unit pen useful if you don't already have one. That way you can increase/decrease your bolus in smaller increments.
 
I thought that was the case! Judging by a previous post of yours.

My 2pence worth: I'd have some glucotabs to get me up to where I want to be and bolus for the meal as normal.
 
I'll try both methods, it's not a common occurrence but a pain when it does happen.
Still what would life be if blood sugars were easy!
