I have decided to create a diet plan for myself as i seriously need to lose weight and I think that my carb intake is not helping and so far I have been able to plan my lunch and tea meals, but having some problems on breakfast meals as everytime i have cereals such as weetabix or porridge I feel hungry again after an hour or so and my BS levels shoot up.

This is what my diet plan looks like.

Lunch: 2 Scrambled Eggs, 1 Ham Slice, 3 Cherry Tomatoes and Half Red Onion / Salad with Chicken Pieces /

Tea: Homemade Tomato Sauce with Mince / Breaded Chicken Breast Fillet with Veg / Veg Stir Fry / Fish Cakes with Veg /

Snacks: Celery Sticks / Cucumber Sticks / Carrot Sticks /

For my breakfast I was thinking a bowl of Coco Pops/Cornflakes/Crunchy Nut with Semi-Skimmed Milk as for some reason they do not raise my blood sugars too much.

I would transfer the scrambled eggs to breakfast. that's what I had this morning. [takes about 70 seconds in my microwave.]
Otherwise it all looks good. All you need to think about is portion sizes. A simple way to help with that is to use a smaller plate. today's dinner plates are about 2.5cm bigger in diameter than the plates of my youth. a 24 cm plate is a good size.
I personally don't eat breakfasst cereals ever. not only aree they high carb, but also very processed.
