Survey for a diabetic research project

As a part of an undergraduate project, me and my groupmates need a few of your thoughts on our project.
We are from BRAC University, Bangladesh, we plan on doing research related to diabetes and its side effects. Hence, we need to know about some physical conditions people with diabetes deal with. We hope in future our research could help people know more about diabetes and its side effects.

Tried but stuck on the second page as I don’t test my BS anymore.

I found that when I looked at the survey that only the fields marked with a * are mandatory/necessary; there are lots that don't impede moving on.

Just my own observation.
