T1 High sugars in the morning

I am 24 have type 1 and have done for over 10 years. I use Novorapid and Levemir.

Every morning I wake up with sugars of around 15. During the day my sugars are completely fine I test a lot and generally go to bed with them at a decent level.

I have tried increasing my Levemir dose and decreasing it but it is still 15 every morning. My normal dose is 16 & 16 morning and evening of Levemir. It wakes me up at around the same time every morning at about 5.30am when I need to go to the toilet and do a test.

It is really frustrating as there seems to be nothing I can do to stop this from happening.

Any ideas?

Jon,

It could be one of two-either dawn phenomenon or rebound hyperglycemia due to hypo's in your sleep, if it is DP then you'll need to increase your night-time levemir to overcome this till waking levels are normal, if its RH then you may need to reduce your evening basal insulin or reduce your meal bolus dose depending on what time you eat, sometimes a small 10g snack can be helpful before bed to stop hypo's. Try setting your alarm 2-3am for a few nights and test your bg to see what's going, if your bg is rising it's DP or if it is falling then its definitely nighttime hypo's to blame.

Thanks Nigel

I have been advised that before and when I have woken up in the middle of the night the readings are normally fine.

As I said I have tried both increasing my Levemir and decreasing it by large and small amounts and neither makes any difference to my morning reading. Always the same time and the same reading in the morning

Jon,

As you are on a split dose of levemir why not try moving your night-time dose forward, for example if you take your evening dose at 8pm try moving it to 10pm, often small changes like this can make a big difference to fasting bg, alternatively ask your dsn for advice as it may be necessary to change your basal insulin to overcome this problem.

Thanks

I will try moving my evening dose later.

Is there anything in the time at which I eat durning the evening. I tend to sometimes snack late in the evening as I work-out each night. I take protein shakes but they are extremely low in carbs 1-2gs

Well if your QA insulin is still working from your evening meal when you go to bed this could explain your morning highs due to nighttime hypo's, I use novorapid and its insulin profile shows that it can still be active 4-5 hours after injecting, however now you mention you are working-out it could be due to the delayed effect of exercising, try eating a 10-20g snack before bed tonight and see what your levels are like tomorrow on waking.
