Please dont think you are going to get your licence automatically swapped... You may well be a person that does... But a great, great many don't.

Is the D202 FORM THE declaration form? I'm slightly confused with the dvla at the minute I received a letter from them saying that they was issuing a 3 year license I had to sign the declaration form and send back with my license in it which I have done but that has been lost in the post so I now have to wait for the declaration form to be posted put to me againYou're confusing me :!: I've only stated the facts of my dealings on this subject from telling DVLA to getting a reply.
The letter I received from DVLA said that medical investigations have been completed and the medical advisor has recommended that my current licence is withdrawn and a new photocard licence will be issued which will be valid for three years.
As I stated earlier I am not applying for a renewal yet. I do expect to have to fill in forms at the end of my three years. DVLA have stated in their letter that they will send me renewal forms three months before the three years expire and my fitness to drive will be looked at again.

You use a DIAB1 to notify DVLA that you are diabetic on insulin.

I did sign the D202 and filled out some other forms so maybe one was a D3Sorry I must have missed that
They are sending me another form through the post as when I originally rang them it was to check to see if they had received it through the post and they hadn't so they are sending me another and I just have to say that I've already sent mine off but it has been lost in the post. Thank you for your reply I was just confused as I thought why do they need more medical info when I have signed the declaration form lol got all my hopes thinking I was okay and then that lol@frith1988 Yes D202 was the declaration form.

If your licence and form have been lost in the post the I do suggest you ring them and discuss it. I know it can take a while to get through to them but I believe they are quite helpful when you do speak to them.
