Temporary Basal Rates on a closed loop

Hi all,

I’ve been on the Medtronic 780g with Simplera for 2 weeks, previously I was using the Medtronic 640g pump with Guardian 3. I’ve been on a pump for 12 years. I do an Aqua fitness class twice a week and previously I used to set a temporary basal rate decreasing the basal to 75% 45 minutes before the class and increased to 125% for 1 hour after, I found this worked for me and kept my levels reasonable. With my new pump it doesn’t allow me to do this presumably because the Smart Guard should adjust it. Last week my BG soared about 2 hours after. I notice in the Smart Guard there is a setting to run a temp basal at 8.4 for 2 hours. Has anyone had on a closed loop tried this. I should add they I’m disconnected from the pump for about 1 hour.

That setting is really for people that do activity while connected to the pump to prevent lows, so not suitable for your situation

The pump team highly recommends not being disconnected from the pump for more than an hour, less if possible

Your rise in bg is lack or insulin due to removing the pump, a temp basal won’t work as the pump is not connected to give that basal, and in the example above raising the target will actually give you less insulin resulting in even higher bg

The only options are to either take a bolus mid activity

When you reconnect after activity you could add a few carbs to the pump to get a small bolus

Or you can let smart guard do its thing
 
Thank you Searley for your reply. I always get out the pool on the low side as the class is pretty full on and I take sips or orange juice during the class. I’ll try a small bolus when I get out the pool, see how that goes.
