So does this still exist? I got told by Boots that i needed to get one, or i would not be able to get my insulin in future. I got one off my GP, looked at it...thought 'this is utterly pointless' and never filled it in. Kept it a few months, then after never being asked to show it i eventually threw it out. Never once been asked for it. Never been given the wrong insulin either, they always show me it and ask me if it's the right one. So far it always has been. If it wasn't i'd say erm...no that's not it! Try again! I can see how it MAY help if you are 90 or mentally impaired, other than that i haven't got a clue what use it could be.
Like others have said, if you're admitted to hospital it's irrelevant information. Any old insulin will do surely. They're pretty sure to check your BG as soon as they bring your file up on the computer, then monitor it if they actually inject insulin.
