Have you read Bernstein's book? (See title in my signature)I'm not sure I have the answer to your question just yet but I think it's because I've been on oral meds for about 14 years now and the last 3 years glucose readings have been steadily rising and I've been very ill.
I only discovered low carbing since joining here, a few weeks ago, it's not easy but I'm working on it.
Going onto insulin....Its a trial... I did discuss with the Dr, that hopefully to avoid putting on more weight, I inject low dose of insulin and stick to healthy lifestyle of eating and excercise to reduce the weight.... I don't know if this'll work...Only time will tell.
Also I'm so desperate to come off gliclazide as it's causing terrible stomach problems

He has wonderful results combining low carbing and small amounts of insulin or other meds, precisely tailored to the individual. It's a great book, and very inspirational.

The kidneys have a mechanism that normally reabsorbs glucose and prevents it being excreted in the urine It works unless the concentration of glucose is very high. (basically the body doesn't normally want to get rid of a valuable fuel)
This drug blocks this mechanism so allowing more glucose to be excreted in this way

Well finally saw the DN yesterday and as soon as I sat down she said, I've discussed your medication with another Dr and we both agree you should continue with metformin and increase the current gliclazide to max 4 tablets daily and start taking twice daily injections of byetta!!!
I reminded her of how unwell I've been since May when I started 2 tablets of gliclazide and I'm desperate to come off this tablet completely. She insisted I have to stay on them. I have constant nausea and bloating, I reminded her how I was violently vomiting a few months ago. Gliclazide has taken away my quality of life.
DN wasn't budging so she phoned another Dr who reluctantly suggested I change to the slow release gliclazide. They said they don't usually prescribe this because it's expensive!!
So tomorrow I Start on 2 x 30 mg SR. gliclazide and then she wants me to increase to maximum 4 daily in few weeks time. Errrmm I don't think so!!
After much debating I told her I'm not starting any of the 3 other injections she suggested they are Bydureon, Lyxumia and Byetta. The side effects are awful.
I said I'd rather start insulin injections but she said you'll put on more weight.
So she's prescribed Forxiga 10mg, a new drug also known as Dapagilflozin... Start this tomorrow, it's supposed to help with lowering blood sugars and suppress appetite.... I think...
Next review is 22 Dec, if stomach is still bubbling...I will ask to come of gliclazide completely. Even though DN is adamant I have to stay on it.

Is there not a nice GP within the practice that you could make an appointment with just to discuss your issues? It's all very well them prescribing a host of standard regime drugs since they aren't the ones taking them. You do have to balance quality of life with improved levels and try and accommodate both. I'd give low carbing a serious stab and see what effect that has as personally I'd not want to be rushing onto insulin just yet if diet can help. I know that several folk on here take Forxiga though it is meant as a substitute for the likes of Glicazide according to the NICE guidelines on the drug on the main website here. Hope the SR helps - I know how you feel with the side effects of drugs as I'm currently fed up with just the Metformin.
are you overdoing the fat bit? I know the diet doctor site and others say unlimited amounts but I don't think you can do that if you are overweight with poor appetite control. I know I could eat a pound of cheese if I let myself and now way would I lose weight then. Maybe start posting what you eat on the what have you eaten thread and we could help. The one thing I would say is you can't high fat and keep the carbs as that is a diet disaster.The lovely Dr whose been seeing me for my stomach problems wasn't at the surgery yesterday, Had she been there it may have been a different outcome. But all's not lost, the next available appointment with Dr is on 22 Dec @ 9.30 am and with DN same afternoon.

I've been on insulin for almost a year now. My initial HbA1c was 112 mmol/l and was told I'd probably be a candidate for insulin therapy. I was a bit sceptical about low carbs as my dietician said I needed them. I did notice I didn't get spikes when I avoided them and now keep carbs to a minimum and have lost some weight.
I know I'm a bit late catching up on this, but one thing I noticed is you mentioned having a 'tiger cob'. I love tiger bread but since I started testing I have found it sends me sky high even though I can eat normal white bread in small quantities with out problems. I don't know what is in the tiger glace but it is not good for me, it might be worth checking for yourself.
